Bridging Care: Community-Clinical Partnerships for Dementia in Hawaiʻi is an in-person gathering dedicated to strengthening collaboration among healthcare providers, community organizations, and public health leaders. Hosted by the Hawaiʻi Dementia Initiative, this morning event focuses on improving referral pathways, care coordination, and integrated services for individuals living with Alzheimer’s Disease and Related Dementias (ADRD). Grounded in Hawaiʻi’s cultural values and community strengths, participants will engage in shared learning and discussion to develop practical strategies that enhance dementia care statewide. Complimentary breakfast begins at 8:00 AM, and valet parking validation is provided.
