This training provides an overview of the Hawaiʻi Med-QUEST 1147 form and its use in assessing an individual’s level of care and support needs. Participants will gain a better understanding of the nuts and bolts of the form, its purpose, and how it can inform care planning, decision-making, and Medicaid long-term care eligibility.

Learning Objectives:

• Articulate the purpose and identify key components of the 1147 form used to assess an individual’s level of care.
• Understand the criteria needed to determine level of care decisions and ability to complete and/or interpret the major sections of the 1147 form.
• Recognize how the 1147 form can contribute to access to patient-centered, medically-appropriate care.

Trainer’s Biography: Desiree Mizuno is a Registered Nurse with over 25 years of experience in healthcare quality improvement, regulatory compliance, managed care, case management, long-term care, acute care, home health, mental health, and performance improvement. She currently serves as Associate Director at Health Services Advisory Group (HSAG), where she leads and oversees Peer Review Organization (PRO) activities, including Long-Term Care (LTC) assessment reviews and Pre-Admission Screening and Resident Review (PASRR) compliance reviews. Prior to her current role, Ms. Mizuno served as a Senior Quality Improvement Manager at ‘Ohana Health Plan where she led quality improvement initiatives focused on preventive health screenings and managed performance improvement projects to improve clinical processes and health outcomes. Her clinical experience includes case management for individuals with intellectual and developmental disabilities at The Arc in Hawaii and direct patient care as a Registered Nurse at The Queen’s Medical Center in both medical-surgical and mental health settings. Ms. Mizuno earned a Bachelor of Science in Nursing from the University of Hawaii at Mānoa School of Nursing and also holds an Associate in Liberal Arts degree from Leeward Community College.

In support of improving patient care, this activity has been planned and implemented by the Hawaii Public Health Institute and the Western Region Public Health Training Center (WRPHTC). The WRPHTC is jointed accredited by the Accreditation Council for Continued Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.
