Health Matters Program for People with Developmental Disabilities

Evidence-based health promotion and disease prevention programs for individuals living with intellectual or developmental disabilities. Health Matters also has educational resources about exercise and nutrition.

Pohai Nani

Good Samaritan Society – Pohai Nani has a many senior living options and hosts a monthly caregiver support group. Services offered at this location: Independent living, Assisted Living, Long-term care, Inpatient rehabilitation therapy, Outpatient rehabilitation therapy, Assisted living respite care. Staff provides activities for the care recipients in a separate room so that the caregiver can participate in the support group.

Tech Savvy Teens

Tech Savvy Teens, Hawaii’s leading initiative focused on equipping middle and high school students with essential technology skills while providing crucial support to our kūpuna (seniors). Older adults can book a session for Technology Tutoring and Coaching.

Tutus House

The Caregiver Foundation

The Caregiver Foundation is a local non-profit organization that works to support caregivers and individuals needing care through a variety of services. They have a range of programs such as Workshops, Caregiver Support Groups, Coordination Services, Daily Money Management, Family Caregiving Planning, Guardianship, Home Organizing Services, Professional Services Support, and Senior Home Cleaning Services. State Kupuna Care Program

The State Kupuna Care Program provides community-based services to older adults so that they can remain at home for as long as possible. These services also support working caregivers to continue their employment, by providing support for their care recipient. Services may include attendant care, home maker, transportation, case management, home delivered meals, chore services, adult day care, respite care, personal care services, and participant-directed care option. The Aging and Disability Resource Centers (ADRC) provide options counseling and can do the assessments to see if a older adult qualifies for the State Kupuna Care Program. If they qualify, the ADRC will make a referral to a provider organization for the services.

Hawaiʻi Family Caregiver Coalition

A group of dedicated caregivers caring for caregivers. They advocate for policies that benefit caregivers in Hawai’i and offer caregiver advocacy training to help people share their stories with local leaders. Hawaiʻi Family Caregiver Coalition hosts an annual conference and participates in national advocacy events. If you’ve got a caregiving question, they’ve got answers!

Project Dana

A local non-profit organization with a mission to provide compassionate support and services to Hawaii’s Kupuna and their caregivers. They offer support groups, caregiver counseling, REACH caregiver 1:1 coaching program, educational presentations for their Caring for the Caregiver program, as well as volunteer service coordination to provide assistance with grocery shopping, light housekeeping, transportation, and visitation.

Positive Approach to Care (PAC)

Positive Approach to Care® enhances life and relationships of those living with brain change by fostering an inclusive global community. Using our talents and abilities to develop awareness, knowledge, and skill with all people, that will transform what exists into a more positive dementia care culture. PAC offers dementia education and training that focuses on skill-building. PAC provides various levels of certification. The PAC Hui is a group of Certified PAC Trainers in Hawai’i. National Parkinson’s Foundation

The Hawaii Parkinson Association (HPA) is a non-profit volunteer group comprised of people with Parkinson’s disease, caregivers, family members, health care professionals, educators and leaders in the business community. They offer support groups, seminars, online resources, and peer-to-peer connections.
