Expanding the Reach of Dementia Programs: Making a Plan and Pivoting as You Learn Through Implementation
Even with the most thoughtful program planning, sometimes plans don’t work as expected when you get to real-world implementation. Reaching and engaging people for dementia programs may require a willingness to adjust course and engage unanticipated partners and participants as you learn through practice. This presentation will showcase the work of two dementia programs that made significant adaptations to their original outreach, recruitment, and partnership plans to support the uptake of their community dementia programs.
OCCK Inc. is implementing the Care Ecosystem care navigator program in rural Kansas. This presentation will focus on adaptations OCCK Inc. made to their partnerships and recruitment techniques when their original plan did not yield desired enrollment levels. OCCK Inc. found and engaged new partners, members of the faith community (nuns) and independent pharmacists, committed to serving people living with dementia and caregivers. They adjusted course by training their new partners in the Care Ecosystem model in their effort to expand their program reach and participant engagement.
Aging & In Home Services in Indiana is implementing the Hospital 2 Home care transitions program. Their presentation will include information on their chosen program, as well as how the team pivoted their implementation plans to infuse new outreach techniques. They will provide insight into the various methods undertaken to engage new health care providers using success stories and training opportunities resulting in increased program participation.
