SUMMARY:Expanding the Reach of Dementia Programs: Making a Plan and Pivoting as You Learn Through Implementation
DESCRIPTION:Even with the most thoughtful program planning\, sometimes plans don’t work as expected when you get to real-world implementation. Reaching and engaging people for dementia programs may require a willingness to adjust course and engage unanticipated partners and participants as you learn through practice. This presentation will showcase the work of two dementia programs that made significant adaptations to their original outreach\, recruitment\, and partnership plans to support the uptake of their community dementia programs. \nOCCK Inc. is implementing the Care Ecosystem care navigator program in rural Kansas. This presentation will focus on adaptations OCCK Inc. made to their partnerships and recruitment techniques when their original plan did not yield desired enrollment levels. OCCK Inc. found and engaged new partners\, members of the faith community (nuns) and independent pharmacists\, committed to serving people living with dementia and caregivers. They adjusted course by training their new partners in the Care Ecosystem model in their effort to expand their program reach and participant engagement. \nAging & In Home Services in Indiana is implementing the Hospital 2 Home care transitions program. Their presentation will include information on their chosen program\, as well as how the team pivoted their implementation plans to infuse new outreach techniques. SUMMARY:Business Strategies to Support Comprehensive Dementia Care
DESCRIPTION:Join the National Dementia Care Collaborative (NDCC) for a free virtual event that will share key lessons and strategies to support comprehensive dementia care implementation. The event will be hosted by NDCC and is made possible by the generous support of The John A. Hartford Foundation. \nDuring the event\, speakers from UCLA and Oschner Health will provide insight into building and sharing the value for dementia programs. \nDavid Reuben\, MD Dr. Reuben is an Archstone Professor at the David Geffen School of Medicine at UCLA and is Director of the UCLA Alzheimer’s and Dementia Care Program and the UCLA Medicare & Medicaid Innovation (CMMI) Guiding an Improved Dementia Experience (GUIDE) program.  In 2012\, Dr. Reuben received one of the first CMMI Innovations Challenge awards to develop a model program to provide comprehensive\, coordinated care for patients with dementia. \nLynn Spragens\, MBA Lynn is the president of Spragens & Associates\, LLC\, and has more than thirty years of experience as a business consultant partnering with clinical innovators in health care with a focus on Geriatrics and Palliative Care. Ms. Spragens has provided strategic planning support to geriatric initiatives such as CAPABLE\, Hospital at Home\, NICHE\, HELP\, GRACE\, and the Medicare Innovations Collaborative (Med-IC)\, as well as several nursing home quality initiatives. She has worked closely with UCLA and the ADC model for more than 7 years. \nJohn Sawyer\, II\, PhD\, ABPP-CN Dr. Sawyer is the Medical Director of Neuroscience Value at Ochsner Health. In this role he helps develop\, measure\, and scale value-based care programs. Specific to value-based care and dementia\, he has been particularly focused on helping health systems rethink the business case around dementia care. This work has helped health systems understand the financial mechanisms to create and sustain critically needed dementia care programs. He is a co-investigator in a NIH/NIA national consortium study of dementia care management led by UCSF. He was recently asked to chair the Alzheimer’s Association newly formed workgroup focused on the business case for care navigation.

SUMMARY:How Medications can Affect the Brain and Behaviors: For Older Adults and Persons Living with Dementia
DESCRIPTION:Catholic Charities Hawaii (CCH) has launched this series of expert speaker panels focusing on Dementia-Related Behaviors that can be challenging for caregivers and providers of persons living with dementia in the community. This workshop will cover medication management for older adults and persons living with dementia. The expert panel presenters will also discuss safely treating depression\, behavioral and psychological symptoms of dementia\, pain and incontinence. \nExpert Panel Presenters:  \nAida Wen\, MD is a Professor at the Dept. of Geriatric Medicine\, John A. Burns School of Medicine at University of Hawaii (UH JABSOM) and is board certified in Geriatrics and Palliative Care. She is the Principal Investigator of the HRSA funded Geriatric Workforce Enhancement Program which strives to enhance geriatric training for all health professionals\, and to integrate geriatrics into primary care. She is Medical Director of Manoa Cottage Kaimuki\, a nursing facility dedicated to dementia care. Passionate about improving dementia care and nursing home care\, Dr. Wen strives to help patients\, providers and caregivers make the journey through this difficult diagnosis as gracefully as possible. \nDr. Camlyn Masuda is an Associate Specialist with UH at Hilo Daniel K. Inouye College of Pharmacy. She received her Pharm.D. degree from Oregon State University and then completed a residency at Kaiser Permanente Health Plan in Honolulu. She continued practicing as an ambulatory care pharmacist at Kaiser Permanente after her residency and then started working for the college in 2014. She is an ambulatory care pharmacist at the University Family Medicine Clinic in Aiea\, part of the UH JABSOM Family Medicine and Community Health Department\, mainly managing adults with diabetes\, assisting with medication recommendations and helping patients find lower cost\, effective and safe medications. \nThe speakers will cover: \n\n\n\nChanges that occur with aging that affect how medications can impact the brain\nWhat medications increase the risk of worsening cognitive function and behaviors\nCommon over-the-counter medications with cognitive side effects\, and alternatives to use instead\nTreating depression safely: nonpharmacological strategies and safer options\nTreating Behavioral and Psychological Symptoms of Dementia safely: non-pharmacological strategies and safer options.\nTreating pain safely: what to avoid\, different options\nTreating incontinence safely: options for evaluation and non-pharmacological strategies to minimize risk of cognitive side effects\nQuestions you can ask the healthcare provider about your medications

DESCRIPTION:Project Dana’s Caring for the Caregiver (C4C) program offers services to family caregivers and soon to be caregivers. The goal of the program is to help alleviate the stress that comes with continuous caregiving responsibilities. Caregivers will have the opportunity to “talk story” about their experiences and current concerns with their peers. The Project also plans outings to various locations and activities for fun and relaxation. The program’s coordinator facilitates the group meetings.
