SUMMARY:Expanding the Reach of Dementia Programs: Making a Plan and Pivoting as You Learn Through Implementation
DESCRIPTION:Even with the most thoughtful program planning\, sometimes plans don’t work as expected when you get to real-world implementation. Reaching and engaging people for dementia programs may require a willingness to adjust course and engage unanticipated partners and participants as you learn through practice. This presentation will showcase the work of two dementia programs that made significant adaptations to their original outreach\, recruitment\, and partnership plans to support the uptake of their community dementia programs. \nOCCK Inc. is implementing the Care Ecosystem care navigator program in rural Kansas. This presentation will focus on adaptations OCCK Inc. made to their partnerships and recruitment techniques when their original plan did not yield desired enrollment levels. OCCK Inc. found and engaged new partners\, members of the faith community (nuns) and independent pharmacists\, committed to serving people living with dementia and caregivers. They adjusted course by training their new partners in the Care Ecosystem model in their effort to expand their program reach and participant engagement. \nAging & In Home Services in Indiana is implementing the Hospital 2 Home care transitions program. Their presentation will include information on their chosen program\, as well as how the team pivoted their implementation plans to infuse new outreach techniques. This session is a powerful\, hands-on art experience that brings our community together! We will use creative expression to share our unique stories and reflect. As we contribute to a collective art piece\, we’ll explore the connections between our individual experiences & the strength we build together. \nLastly\, there will be A time to connect! Our Lorenzo’s House community will gather in breakout rooms and youth leads will guide informal activities to get to know one another and have fun with peers worldwide walking a similar journey.

DESCRIPTION:“Music and Memories” is a Benefit Concert for the Alzheimer’s Association of Hawaii featuring The Makaha Sons\, Henry Kapono and Friends. This concert is a special tribute to Dr. John Henry Felix. \nThe Makaha Sons have received numerous proclamations and awards both in Hawai’i and in the continental U.S. for their continuous and undying efforts in perpetuating the Hawaiian culture through their music. Their peers in music have also acknowledged their efforts by awarding them countless “Na Hoku Hanohano Awards” for their timeless recordings and steadfast commitment to the music of Hawai‘i. They have released 22 CD’s\, thus far.
