SUMMARY:Research Workshop on Comprehensive Dementia Care Models: GUIDE and Other Models
DESCRIPTION:The purpose of this one-day virtual workshop is to bring together experts in the field to include current NIH grantees and future NIH applicants focused on the GUIDE model and/or other comprehensive dementia care models. The workshop will discuss differential access to GUIDE\, impact of payment models in quality of care or access to care\, and additional data that could be beneficial to researchers or GUIDE model healthcare providers. Furthermore\, the workshop will address comprehensive dementia care models’ utility and challenges across the full disease continuum. The workshop participants will identify potential research opportunities for dementia care coordination models and discuss available and needed data to address research gaps. Outputs should also inform researchers\, patients\, families\, health systems\, and federal partners working with GUIDE and other dementia care coordination programs.

SUMMARY:Training on 1147 Form: Assessing Level of Care
DESCRIPTION:This training provides an overview of the Hawaiʻi Med-QUEST 1147 form and its use in assessing an individual’s level of care and support needs. Participants will gain a better understanding of the nuts and bolts of the form\, its purpose\, and how it can inform care planning\, decision-making\, and Medicaid long-term care eligibility. \nLearning Objectives: \n\nArticulate the purpose and identify key components of the 1147 form used to assess an individual’s level of care.\nUnderstand the criteria needed to determine level of care decisions and ability to complete and/or interpret the major sections of the 1147 form.\nRecognize how the 1147 form can contribute to access to patient-centered\, medically-appropriate care.\n\nTrainer’s Biography: Desiree Mizuno is a Registered Nurse with over 25 years of experience in healthcare quality improvement\, regulatory compliance\, managed care\, case management\, long-term care\, acute care\, home health\, mental health\, and performance improvement. She currently serves as Associate Director at Health Services Advisory Group (HSAG)\, where she leads and oversees Peer Review Organization (PRO) activities\, including Long-Term Care (LTC) assessment reviews and Pre-Admission Screening and Resident Review (PASRR) compliance reviews. Prior to her current role\, Ms. Mizuno served as a Senior Quality Improvement Manager at ‘Ohana Health Plan where she led quality improvement initiatives focused on preventive health screenings and managed performance improvement projects to improve clinical processes and health outcomes. Her clinical experience includes case management for individuals with intellectual and developmental disabilities at The Arc in Hawaii and direct patient care as a Registered Nurse at The Queen’s Medical Center in both medical-surgical and mental health settings. Ms. Mizuno earned a Bachelor of Science in Nursing from the University of Hawaii at Mānoa School of Nursing and also holds an Associate in Liberal Arts degree from Leeward Community College. \nIn support of improving patient care\, this activity has been planned and implemented by the Hawaii Public Health Institute and the Western Region Public Health Training Center (WRPHTC). The WRPHTC is jointed accredited by the Accreditation Council for Continued Medical Education (ACCME)\, the Accreditation Council for Pharmacy Education (ACPE)\, and the American Nurses Credentialing Center (ANCC)\, to provide continuing education for the healthcare team. The planners and presenters for this training have no declared conflicts of interest.

SUMMARY:From Awareness to Action: Community Health Workers Supporting Brain Health and People living with Dementia
DESCRIPTION:Join the BOLD Public Health Centers of Excellence\, the Alzheimer’s Association\, and the National Association of Community Health Workers (NACHW) to explore how CHWs can serve as trusted dementia resources in the communities they serve. \nCommunity health workers are uniquely positioned to support brain health across the full continuum of prevention from reducing risk before symptoms appear\, to recognizing early warning signs\, to supporting families already navigating a diagnosis. Activities like promoting risk reduction strategies such as cardiovascular health\, social engagement\, and sleep (primary prevention)\, connecting families to dementia specialists for early evaluation (secondary prevention)\, and linking households to ongoing care and support resources (tertiary prevention) all show the vital role CHWs play in brain health across their communities. \nThis webinar will equip CHWs with practical tools\, actionable strategies\, and foundational knowledge to confidently contribute to healthy brain efforts at every stage wherever they work. \nDuring the session\, you will: \n\nLearn about brain health\, dementia\, and resources to share with community members.\nUnderstand how brain health and dementia care relate to CHW core competencies and goals.\nLearn how CHWs can facilitate direct referrals to dementia specialists and Alzheimer’s Association resources.\nAccess ready-to-use tools and resources to support community outreach.

SUMMARY:Dementia Friends Champion Training
DESCRIPTION:Dementia Friends is a global movement that is changing the way people think\, act\, and talk about dementia. By helping everyone in a community understand what dementia is and how it affects people\, each of us can make a difference for people touched by dementia. This Hawaiʻi train-the-trainer workshop will cover the Dementia Friends content and teach participants to lead Dementia Friends informational sessions in their communities. Participants will receive the Dementia Friends resource materials and be empowered to become Dementia Friends Champions. This is not a clinical training about dementia.  \nPresenter: Christy Nishita\, Ph.D. is a Gerontologist at the University of Hawaiʻi Center on Aging. She received her Ph.D. in Gerontology from the University of Southern California. Her focus is on improving community-based long-term care\, dementia care systems\, workforce development in aging\, and intergenerational programming. She is the co-founder and co-coordinator of the Kupuna Collective\, a statewide network of over 200 kupuna-serving organizations who are incubating new thinking and action in the aging network. She is the consultant for the Mayor’s Age-Friendly Honolulu Initiative\, which is creating more inclusive and equitable communities in Honolulu. Dr. Nishita is also leading policy and programmatic strategies to develop a more age-friendly and dementia-capable workforce in Hawaii.
