DESCRIPTION:Join the National Dementia Care Collaborative for a free\, virtual event on Thursday\, July 9th at 9AM HST / 3PM EST/ 12 PST. BRI Care Consultation™ Learning Collaborative will present The Role of Community Health Workers in Comprehensive Dementia Care. \nCommunity Health Workers are increasingly recognized as valuable members of interdisciplinary care teams\, helping improve access to care\, strengthen community connections\, and support individuals living with dementia and their family and friend caregivers. Marquita Rockamore\, MA\, GCDF\, C-CHW\, Executive Director\, Health Industry Solutions Training Academy\, in the Workforce Innovation at Cuyahoga Community College will provide an overview of the education and training that prepare community health workers for the workforce\, as well as an outline of their appropriate scope of practice and the level of supervision and collaboration needed to ensure quality care. \nThe presentation will also feature Laura Gano\, PhD\, MPH\, Director of Operations\, Indiana University School of Medicine’s Geriatrics Division\, who will overview the Aging Brain Care (ABC) program\, an evidence-based dementia care model that leverages community health workers as care navigators to deliver ongoing care coordination\, caregiver support\, symptom monitoring\, and education under clinical supervision. \nFinally\, participants will hear firsthand from Kelsie Owen\, MPA\, Vice President of Nutrition & Population Health at Aging and In-Home Services of Northeast Indiana\, an Area Agency on Aging that implements the Aging Brain Care program using community health workers\, including their insights into training and implementation\, the program impact\, and lessons learned. DESCRIPTION:This Positive Approach to Care (PAC) Approved workshop helps learners understand and recognize the differences in normal and not normal aging. The learner will develop better observational skills to recognize and intervene effectively when behavioral challenges occur. In this webinar\, Amazing Place and OPICA Adult Day Program & Counseling Center will share insights into innovative programs and strategies that have proven successful in supporting people living with dementia and their family caregivers. \nAmazing Place will highlight its innovative efforts to recruit and engage a multigenerational workforce. Through its STARS (Student Training in Alzheimer’s & Respite over the Summer) program and Legacy Workforce model\, the organization creates meaningful opportunities for both individuals beginning their careers and those pursuing new opportunities after retirement. \nOPICA will share strategies that support the sustainability of its adult day programs through an extensive multidisciplinary internship program and innovative fundraising initiatives. The presentation will highlight the vital role interns play in engaging and supporting individuals with early memory loss. DESCRIPTION:The purpose of this one-day virtual workshop is to bring together experts in the field to include current NIH grantees and future NIH applicants focused on the GUIDE model and/or other comprehensive dementia care models. The workshop will discuss differential access to GUIDE\, impact of payment models in quality of care or access to care\, and additional data that could be beneficial to researchers or GUIDE model healthcare providers. Furthermore\, the workshop will address comprehensive dementia care models’ utility and challenges across the full disease continuum. The workshop participants will identify potential research opportunities for dementia care coordination models and discuss available and needed data to address research gaps. Outputs should also inform researchers\, patients\, families\, health systems\, and federal partners working with GUIDE and other dementia care coordination programs.

DESCRIPTION:Dementia Friends is a global movement that is changing the way people think\, act\, and talk about dementia. By helping everyone in a community understand what dementia is and how it affects people\, each of us can make a difference for people touched by dementia. This Hawaiʻi train-the-trainer workshop will cover the Dementia Friends content and teach participants to lead Dementia Friends informational sessions in their communities. Participants will receive the Dementia Friends resource materials and be empowered to become Dementia Friends Champions. This is not a clinical training about dementia.  \nPresenter: Christy Nishita\, Ph.D. is a Gerontologist at the University of Hawaiʻi Center on Aging. She received her Ph.D. in Gerontology from the University of Southern California. Her focus is on improving community-based long-term care\, dementia care systems\, workforce development in aging\, and intergenerational programming. She is the co-founder and co-coordinator of the Kupuna Collective\, a statewide network of over 200 kupuna-serving organizations who are incubating new thinking and action in the aging network. She is the consultant for the Mayor’s Age-Friendly Honolulu Initiative\, which is creating more inclusive and equitable communities in Honolulu. Dr. Nishita is also leading policy and programmatic strategies to develop a more age-friendly and dementia-capable workforce in Hawaii.

DESCRIPTION:Palliative Care Through Song: An Arts-Based Caregiver Education Course is an interactive\, two-session course designed to provide family caregivers with practical tools and knowledge to support loved ones experiencing serious illness. Through original music\, visual art\, storytelling\, discussion\, and evidence-based education\, participants will explore approaches to comfort\, communication\, nutrition and safe swallowing\, symptom management\, advance care planning\, and caregiving at the end of life. \nAcross two 2-hour sessions\, participants will also explore common misconceptions about hospice\, palliative care\, and pain management; caregiver well-being and resilience; and resources that can help individuals remain safely and comfortably at home. Developed by Dr. Ritabelle Fernandes\, Professor of Geriatric Medicine and creator of the Returning Home educational music project\, the course translates complex healthcare concepts into accessible\, memorable\, and practical lessons caregivers can apply in their own lives. No healthcare background is required. \n\n\nLearner Outcomes\nBy the end of the course\, participants will be able to: \n\nDescribe the purpose and basic principles of palliative care and hospice and recognize key differences between them.\nIdentify practical strategies to support comfort\, safe eating and swallowing\, and symptom management during serious illness.\nExplain the importance of advance care planning and communicating healthcare wishes before a crisis occurs.\nRecognize common misconceptions about pain management and medications such as morphine.\n Identify approaches and community resources that can support individuals and families providing care at home.\nRecognize common experiences and caregiving needs that may arise during the final stages of life.\nIdentify strategies for supporting caregiver well-being\, grief\, resilience\, and self-care.\n\n\nInstructor: Dr. Ritabelle Fernandes \nWhen: Thursdays\, 5:00–7:00 PM\nDates: October 29 & November 5\nDuration: 2 weeks\nCourse Fee: $10 \nCourse Materials: $34
